Member Attendance Make-Up Form

This Form is to be completed by ALL Members  of the Rotary E-Club of Canada One, qualifying for attendance or Make-Up.

All Fields require completion before the form will be accepted. You will receive a confirmation by e-mail. If you do not receive the confirmation, contact the club secretary immediately at reccowaugh@gmail.com 

Membership Attendance Make-Up

Members of the Rotary E-Club of Canada One are required to commit to a minimum of 9 to 12 hours Rotary service per quarter.  Qualified Service includes participation in the service activities of your or other Rotary Clubs; Service in your community, or joining in service activities together. 

Activities for Attendance or Make Up, must be reported 4 weeks before or after a missed meeting.

Submit a separate form for each attendance.

Members are required to contribute to the weekly meeting, by serving as greeter, submit ideas for "A Little Humor, Inspirational Moment, Food for Thought, Rotary Minute, or suggestions or submission of material for the weekly speaker program and well as regular donations to the club in lieu of a meal.

Please send your submission for program material to our Program Chair.

A minimum donation of $10.00 for attendingour meetings is suggested from all members. 

These donations allow us to do good in the world through projects.
