Cantex Senior Communities
Cantex, of which Kenny Blanda's Magnolia Manor is a part, is initiating a transitional care program with transitional care coaches. The average 72-year-old senior takes 23 medications a day and has 37 health encounters a year. Transitional care is a patient-centered way of integrating the different health care provisions to the benefit of the patient.
