Rotary District 6600 would like you to take a brief survey about COVID vaccines.
Disease prevention is one of Rotary’s areas of focus. To that end, I respectfully request you to consider immunization, if you haven’t already. As an incentive, a fellow Rotarian in District 6600 will donate funds to each club based on their club’s vaccination rate as of September 30, 2021, if members complete the survey linked below.
The survey is highly confidential – only Rotarians needed to tally results will have this information, and it will be held in the strictest of confidence.
If you select YES, please include your name and your club name. PREFER NOT TO SAY will be counted as a NO.
Here are the incentive details:
• For every YES response that includes the Rotarian’s name and club name, that club will receive $100, times number of members vaccinated, times the percentage that the vaccinated members are of the club’s total membership.
• For example, if there are a total of 50 club members and 32 are vaccinated, the 32 vaccinated members represent 64% of the club
o $100 x vaccinated members x vac rate = $100. X 32 x .64= $2048
o 32 is the number of members who got vaccinated
o The 0.64 is the percentage of club members who got vaccinated
Funds will be donated to club’s Foundation or used by the club for their Rotary grants.
