Program

Program: PACE:  A Model of Care for the Elderly
Presented by Leslie Mader

Leslie spoke to our club on February 12th, about a new  care option for older adults in Boulder and Weld Counties.   The program, called PACE (Program of All-Inclusive Care for the Elderly) is a national program.  The first PACE program started in the 1970's in San Francisco.  Since that time there are  now 122 programs operating 233 PACE centers in 31 states.  The PACE program for Boulder/Weld counties is a division of TRU Community Care.  The PACE program opened March 1, 2017 with a brand new day center located in Lafayette.  PACE is a holistic model of care, working with an individual’s Medicare and/or Medicaid benefits to provide an all inclusive care package.  The client continues to live in their own home and receives the needed medical care, social support through the day center and transportation.  The care plan is individual to meet the client needs and desires.  For more information about this program, go to the website at www.pace.trucare.org.
