Please answer four questions with your registration as follows:

1. Club Name – for example “Willow Grove”, NOT “the Rotary Club of Willow Grove”;
2. District number – “7430”, NOT “D7430” or “District 7430”;
3. Role – PE, AG, Training Leader, Staff, SAA, Presenter, Speaker – answer as written in the question – NOT “President Elect”, etc. ; and
4. Number: “1”.
