Rachael talked with us about advanced care planning

Rachael handed out the Five Wishes Document.  She recommends filling out the document at the age of 18.  The purpose is to direct what you want done if you can’t speak for yourself and who you want to speak for you.

HIPPA is strict, so you need to formally list an agent who will make information available to the family.

The Five wishes document helps people walk through the thought process.

JourneyCare was formed in 2015 as a result of the merger of three highly respected nonprofit community-based healthcare providers, Horizon Hospice & Palliative Care, JourneyCare, and Midwest Palliative & Hospice CareCenter, each with more than 30 years of experience. The merger expanded access to a wide range of services and care to even more patients and families across 10 counties in the Chicago area and northern Illinois.

JourneyCare helps people facing serious illness live life to the fullest, with dignity and without pain, on their own terms. We care for the whole person—body, mind and spirit—not just the disease, and work together with the patient, family and other care partners to create a customized care plan that supports and respects their goals, preferences and choices.

Care is provided wherever the patient calls home…a private residence, senior living community, a long-term care facility or hospital…and a team of trained nurses is available 24 hours a day, seven days a week.  If pain and symptoms become too difficult to manage at home, around-the-clock care can be provided in one of our five inpatient Hospice CareCenters.

With more than 1,000 team members and over one thousand volunteers, many of whom built our organization from the ground up, JourneyCare has a deep understanding of our community. We are committed to education, for both community members and healthcare professionals. JourneyCare also partners with area colleges and universities to offer accredited fellowships, residency programs and student rotations.
