Achieving the status of being a 100% Paul Harris club is quite the accomplishment, maintaining that status proves to be a challenge as we continue to increase our membership. The opportunity to receive your first Paul Harris, give the gift of a Paul Harris to someone special or add another Paul Harris to your name is once again made easier, thanks to a very generous anonymous donor. Please read the rules to the 2013 – 2014 Paul Harris Match Point Program, then download the Contribution Form/The Rotary Foundation form located under the “DOWNLOAD” section of the club’s website

Paul Harris Match Point Program Rules 2013-2014

District 6440 Foundation Chair PDG Mike Ericksen is pleased to announce that, once

again, a most generous anonymous donor is making 100,000 points available to match

unrestricted cash donations to The Rotary Foundation Annual Programs Fund. This is a

great opportunity for Rotarians to become Paul Harris Fellows, to honor a special

person with Paul Harris Recognition, or to add another Paul Harris level. Do take

advantage of this generous donation and support the wonderful work of Rotary

International.

Here are the rules to qualify for 500 match points:

1. Donations to The Rotary Foundation must be in the amount of $500 or more, in

a single payment or by accumulation commencing July 1, 2013. Larger

donations are encouraged. Donations of $1000 or more will be matched by

1000 points allowing for a second Paul Harris Recognition for the donor or his

designee.

2. Donations must be designated to the Annual Programs Fund.

3. Payment to The Rotary Foundation may be made by check or credit card by

donor or club (on behalf of donor) or a combination thereof. If payment is made

by credit card, you may submit to Ann Hughes by email (annshughes@att.net)

and it will be completed and forwarded to RI by email. This allows for almost

immediate processing. Use snail mail if you are not comfortable with this

option for security reasons.

4. Do NOT send payment or forms directly to the Rotary Foundation.

5. If submitting Global Contribution Form 123-EN-(908), complete sections I, II, III,

and IV. LEAVE SECTION V BLANK. Submit completed form with payment to:

Ann Hughes, 15202 MacIntyre Lane, Woodstock, IL 60098, annshughes@att.net,

phone 815-338-4046. A pre-populated version of this form is available from

RI online. Leave Section V blank.

6. If submitting Contribution Form 123-EN-512 AND Recognition Transfer Request

Form 102-EN-(908), complete all of contribution form, leave Section 2 BLANK

of Recognition Transfer Request Form. This is the blank version available from

RI online and consists of two pages. Fill in both, please, leaving Section 2 blank

of Recognition page.

n.b. ONLY when using the two page online forms: if the Paul Harris recipient is NOT

the donor, you must fill out (and the donor MUST sign a second Transfer Request Form

102-EN-(908) to transfer his/her points from donation to the recipient. (The first copy

transfers the match points to the recipient.)
